Group participation

As part of participating in the 'Building Your Neurodivergent Identity' group program, Sarah Conway Psychology needs to collect and record personal information from you. This information collected will include (but may not be limited to) your name, contact details, medical history, employment status and other relevant information as appropriate.

The collection of this information is a necessary part of participating in the group and allows the group to run effectively. Your informed consent must be given prior to commencement of the group. You may withdraw from the group at any time if you wish.

 

Privacy and Confidentiality

Your personal information is gathered both during the assessment process and throughout the program, in order to provide a thorough service. All of this information is kept securely and, in the interests of your privacy, used only by your psychologist and the authorised personnel of the practice (as necessary).

Your psychologist is registered with the Australian Health Practitioner Regulation Agency (AHPRA) and it is a requirement that all psychologists follow strict guidelines for professional conduct that is in line with AHPRA and the Australian Psychological Society (APS) Code of Ethics.

Your personal information is retained in order to document what occurs during sessions and enables the psychologist to provide a relevant and informed service to you. You are entitled to access your personal information kept on file at any time.

 

Limits to Confidentiality

All personal information gathered by the psychologist during the provision of the psychological service will remain confidential except when:

  1. It is subpoenaed by a court; or
  2. Failure to disclose the information would, in the reasonable belief of Sarah Conway Psychology, place you or another person at serious risk to life, health or safety; or
  3. Your prior approval has been obtained to:
  • provide a written report to another professional or agency. e.g. GP, school or a lawyer;
  • discuss the material with another person, e.g. a parent, employer, health provider, referral partner; or booking and reception management service, or disclose the information in another way; or
  • You would reasonably expect your personal information to be disclosed to another professional or agency (e.g. your GP) and disclosure of your personal information to that third party is for a purpose which is directly related to the primary purpose for which your personal information was collected; or
  • Disclosure is otherwise required or authorised by law; or
  • Consulting with colleagues, or in the course of supervision, in order to provide you with a better service. In this instance, I am required to conceal your identity and any associated parties involved, and to preserve your privacy at the utmost professional manner in accordance with the APS Code of Ethics.

 

Group Confidentiality:

It is important that you keep all discussions within the group strictly confidential and do not disclose information about others, including their identities to those outside of the group.

 

Consequence of Not Providing Personal Information

If you do not wish for your personal information to be collected in a way anticipated by this consent form, Sarah Conway Psychology may not be in a position to provide the service to you.

 

Fees and Payment:

The total cost of participating in the program is $750. You may choose to pay via instalments of $125 per week, or via one upfront payment prior to the group starting.

Regardless of your choice of payment option, you are required to pay for all 6 sessions of the group, whether you attend every session or not.

Any additional administration, outside of expected administration within the program structure, will incur additional costs. These will be discussed with participants/caregivers prior to completion.

 

Cancellation Policy:

If you are unable to attend a group session, please advise Sarah via email. Where possible, sessions will be recorded to allow for participants to catch up on any content they may miss. However please note that recording of sessions can only happen if ALL participating members provide consent. Refunds cannot be provided for missed sessions.

  • 6 x weekly Zoom sessions from 10am-12pm starting Thursday Oct 16th until Thursday Nov 20th
  • Recordings of each session so you can catch the replay if you're unable to make it live
  • 100+ page workbook delivered to your doorstep before we begin
