Please complete this form to register for a group program at Sarah Conway Psychology

Simply select which group you are interested in and provide some additional information about you or your child. Please complete a form for each child if you have multiple children who will be attending a group.

Once you complete the form, you will receive an email from that includes an intake form and payment options for the group. You are welcome to pay in one upfront payment, or via weekly instalments. Please note that your/your child's place in the group is not confirmed until the intake form and payment details have been received.
